The following items must be completed and turned into the PDO Director for your child to be enrolled in our program.

  • Signed Acknowledgement of Parent Handbook
  • Current Immunization Record
  • Consent Forms for Emergency Medical Authorization, Fieldtrips, and Water Days
  • Pick up Authorization Information
  • Child Information Sheet
  • Registration Fee Paid


Our enrollment period is held in April for Summer, Fall and Spring Sessions. However, we accept children year-around based on classroom availability. A registration packet can either be mailed to you or can be picked up from our PDO Director, Tessa Armes.
