- Current Immunization Record
- Consent Forms for Emergency Medical Authorization, Fieldtrips, and Water Days
- Pick up Authorization Information
- Child Information Sheet
- Registration Fee Paid
Our enrollment period is held in April for Summer, Fall and Spring Sessions. However, we accept children year-around based on classroom availability. A registration packet can either be mailed to you or can be picked up from our PDO Director, Tessa Armes.
